Requirements of the Customer

When you submit your Public Records Request form, please provide us with the following info:

  • Full Name - Provide us with your full name so we know who is reaching out to us.
  • Email Address - Make sure to give your up to date, most frequently used email address so we can get back to you in a timely manner.
  • Phone Number - In case we need to reach out to you via phone, provide us the best number to reach you at. We will only contact you during business hours.
  • Detailed Message - Give us a detailed message so we know who exactly to get you in touch with, and we can respond with the best information possible.
